Goa Mediclaim Scheme: The Goa Mediclaim Scheme was launched by the Health Department, Government of Goa. This scheme applies to every permanent resident residing in Goa for 15 years.
The objective of this scheme is to provide financial assistance for super-specialty treatments not available in the Goa government hospitals, such as dialysis, plastic surgery, ICU care, angiography and angioplasty, open-heart surgery, kidney transplant, neurosurgery, radiotherapy and chemotherapy for cancer, cochlear implant, and bone marrow transplant.
Eligibility criteria of the Goa Mediclaim Scheme:
The eligibility criteria of the Goa Mediclaim Scheme are:
- The applicant must be a permanent resident of Goa.
- The applicant must have resided in Goa for 15 years, be listed in the voter list, and have a permanent ration card.
- The applicant’s annual income should be less than ₹1,50,000/-. However, retired state government employees are eligible, irrespective of their income.
- The applicant needs a facility that is not available in any government hospital in Goa (including Goa Medical College, Bambolim, and District Hospital). However, for NICU, ventilatory support, and kidney dialysis, the patient can go to any of the listed hospitals, even if those facilities are available in a government hospital.
- The applicant must have any illness/disease that requires one of the following treatments: Continuous Ambulatory Peritoneal Dialysis (CAPD), Dialysis, Plastic Surgery, Intensive Care Unit (ICU), Neonatal Intensive Care Unit (NICU), Pediatric Intensive Care Unit (PICU), Trauma Care, Coronary Artery Bypass Grafting (CABG) / Percutaneous Transluminal Coronary Angioplasty (PTCA) (Angiography and Angioplasty), Open Heart Surgery (Valve Replacement, Mitral Valve Replacement (MVR), Ventricular Septal Defect (VSD), Atrial Septal Defect (ASD)), Kidney Transplantation, Neuro Surgery, Radiotherapy / Chemotherapy (Cancer), Cochlear Implant, Spastic Child, Cerebral Palsy, skeletal deformities and bone marrow transplantation.
Mode or procedure to avail the benefit of the Goa Mediclaim Scheme:
In order to avail this facility, the concerned person/individual has to apply to the Director of Health Services, Panaji, Goa, in the prescribed Form www.goa.gov.in/wp-content/uploads/2022/05/Goa-Mediclaim-Scheme.pdf C & D along with the required documents to avail the scheme.
Documents required for the Goa Mediclaim Scheme:
- To receive the insured amount for hospitalization and medical expenses, the policyholder must submit the documents listed below:
- Form C and Form D.
- Recent passport-size photograph.
- Copy of the electoral card of the policyholder or insured person.
- Medical Certificate with Form A. If the patient has to undergo treatment in a listed hospital located outside the state of Goa, then Form A must be attested by the Medical Superintendent of Goa Medical College.
- Form 4 with the individual’s income certificate.

Notes:
The scheme provides coverage for medical expenses during and even after hospitalization. With the Goa Mediclaim Certificate, a resident of Goa can seek treatment at any of the listed hospitals in the state and some hospitals located outside the state of Goa.
